How the service can help

Forces For Change is a scheme which helps connect ex-service veterans with the armed forces community and highlight the services available to them.

We aim to help people from armed forces communities feel more connected to their local area and understand the wider support on offer. There is already excellent support in South Tyneside for the armed forces community and we'd like to help link you in with this existing support.

First Contact Clinical will offer coaching to individuals who have been identified as needing additional support.

The scheme aims to:

  • Identify what is affecting an individual's wellbeing and how they would like things to be different.
  • Build knowledge, motivation and confidence to make changes.
  • Set and achieve personal goals to improve wellbeing.
  • Connect with people, activities, groups and opportunities in the area.
  • Develop strategies for managing mental and/or physical health conditions.
  • Navigate/coordinate your support if multiple services are involved.

Location

We have a team in every GP surgery in South Tyneside.

Your appointment frequency and length are decided by you.

Testamony

As a former Lance Corporal in the British Army, I truly understand the difficulties transitioning into civilian life which is why I think it's great that First Contact Clinical is now supporting veterans of all ages, whether it's integrating into a new community or supporting with a lack of confidence in civilian life.

I truly believe we can help make positive behaviour changes for veterans and I am looking forward to seeing the positive impact I know we can have.

I personally would have greeted the support with open arms when leaving the army.
