Adult Social Care is rapidly changing in South Tyneside. We are continuing our exciting journey of change which focuses on supporting our residents to live healthier lives longer, promoting independence and wellbeing, whilst delivering personalised services in partnership with them.
Working with people and their families and carers' remains a central part of what we do to ensure that they are able to make choices and decisions about their lives. Focusing upon individual outcomes; putting the person in control and making the best use of our resources are at the heart of our Adult Social Care Strategy.
Our aim is to work with all our partners and deliver Practice to be Proud of to support the people of South Tyneside to Live Better Lives. To enable us to do this, we are changing our practice to have conversations to find out what really matters to the people we work with and we are getting to know what there is in our communities, so we can link people with the right support.

This is a challenging, yet rewarding position and you may be assigned to one of the above teams. You will undertake a range of interventions to ensure adults with care and support needs and their families and carers' achieve their desired outcomes.
