The team works with those children and young people who have a permanent and substantial impairment, typically needing input from a number of agencies on a lifelong basis.
The service provides an assessment on the needs of the family and child / young person, and finds the services that would be most suited to support the child / young person and their family.
The role of the team is to:
- receive appropriate referrals
- ascertain whether or not the criterion for you is met - feedback will be given to the referrer within 24hrs on receipt of the information
- assess need and provide a care plan and services if appropriate
- plan for a transition to adult services
How to get support from the service
If you feel that your child meets these criteria you can contact the Contact and Referral Team direct, or you can ask a professional you are involved with to make a referral on your behalf.
The duty worker at the team will take the information and decide whether a discussion with the Children and Adults Disability Service is needed.
