Specialist Health Visitor for Children with Additional Support Needs (0-19 years)
A specialist public health practitioner who has expertise in child development and disability.
The role involves working with you and professionals from health (including paediatricians, health visitors, speech and language therapists, occupational therapists, physiotherapists, school nurse, family nurse partnership etc.), and agencies to make sure your child's care is provided in a coordinated, timely manner, and is centred on the individual needs of your child and family.
This is especially important at key transition stages such as starting or changing nursery or school, and moving on into adulthood.
